All Smiles Dental Membership

Membership available only to patients without dental insurance.  Annual fee and benefits apply to a single individual, and are not transferrable among family members, or to another provider. Discount excludes implants, Invisalign® and any services referred to outside specialist. Plan year runs 1 year from the date of enrollment and automatically renews annually on the date of enrollment. Unused benefits expire and do not rollover at the end of plan year. Patient is responsible for scheduling and attending appointments. 48 hours notice is required for scheduled hygiene appointment cancellations. Failure to attend a hygiene appointment and hygiene appointment cancellations with less than 48 hrs notice will forfeit the hygiene visit and payment will be required at the time of visit when rescheduled. Patient may cancel plan at any time.  This plan is designed to prioritize oral health; therefore, no refunds will be issued under any circumstance.
